Create Risk Assessment Template

The Create Risk Assessment Template page allows governance and compliance leads to build custom assessment templates tailored to their organization's specific AI and data risk requirements.

Purpose

While RexCommand includes globally standardized templates aligned with governance frameworks such as NIST AI RMF and GDPR, many organizations have internal policies, industry-specific concerns, or audit requirements that require customized risk evaluations.

This builder enables users to:

  • Create organization-specific assessments

  • Group questions into logical sections and subsections

  • Customize question types and input formats

  • Define which templates apply to AI systems, datasets, or both

  • Ensure assessments reflect internal accountability and compliance criteria

Step-by-Step Guide to Using the Template Structure Builder

1. Fill in Template Details

Start by entering:

  • Template Name – A clear, descriptive name (e.g., “Internal Model Review Assessment”)

  • Description – A short explanation of what this template covers and when it should be used

  • Applicable To – Choose whether this assessment applies to AI systems, datasets, or both

2. Add a Section

Click Add Section to create a major grouping within your assessment. This might represent a theme such as "Model Governance" or "Data Privacy." Sections help break the template into organized categories.

3. Add a Subsection

Click Add Subsection inside your section to further organize questions. For example, a section on "Model Governance" might include subsections for "Model Transparency" or "Human Oversight."

4. Add a Question

Each subsection can contain one or more questions. To add a question:

  • Click Add Question

  • Choose a Question Type:

    • Short Text – Open-ended, single-line responses

    • Long Text – Open-ended, multi-line responses

    • Select One (Radio) – A list where users choose one option

    • Select Many (Checkboxes) – A list where users can select multiple options

  • Enter the Question Text – This is what the assessor will be asked

Repeat as needed to build out your full assessment structure.

5. Organize and Manage Content

You can reorder sections, subsections, or questions using drag-and-drop or the arrow next to the trash can. Use the red trash icons to delete any unwanted elements. 

You can also hide and unhide sections as you complete and work on them. 

6. Preview the Template

Click Preview in the top right of the builder to see what the final assessment will look like for users. This helps verify layout and flow before saving.

Notes

  • Templates built here will become available when users launch new risk assessments

  • You can return and edit custom templates at any time

  • Global templates cannot be modified, but you can create your own versions inspired by them
