The Incident Details Page displays the full record of a reported incident. This includes information captured during the initial report, any updates made throughout the incident lifecycle, and relevant associations with AI systems and datasets.

This page allows users to manage the progress of the incident, update its status, and ensure the appropriate context and ownership are in place.

Purpose

Use this page to view, review, and update an individual incident. Whether the issue is under investigation, resolved, or escalated, this page provides a complete overview of all activity tied to the incident.

Key Actions

  • Edit Incident – Click the Edit Incident button to modify the incident’s content, including metadata, descriptions, and linked fields.

  • Update Status – Use the Update Status button to move the incident forward through phases like Investigating, Contained, Remediated, or Closed.

  • Attach Assets – From the Related Assets tab, you can link the incident to relevant AI systems and datasets. This provides visibility into which systems were impacted.

  • View Sections – Review structured information in the following areas:

    • Description

    • Containment Actions

    • Root Cause Analysis

    • Corrective Actions

    • Harm and Compliance Impact

    • Retest Results and Lessons Learned

Additional Tabs

  • Related Assets – Displays connected AI systems and datasets affected by this incident.

  • History – Shows a log of changes made to the incident over time.

  • Comments – Supports internal discussion and collaboration among stakeholders.

Notes

  • Use this page to ensure all key fields are completed before closing the incident.

  • Keeping the incident status up to date helps coordinate response across teams.

  • Linking datasets and systems enhances traceability and supports audit readiness.
